If you are absent from work due to sickness, you may be entitled to statutory sick pay (SSP) or Occupational sick pay in accordance with your contract. However, you must comply with the sickness absence reporting procedure set out in your contract and the CHoC Sickness Policy. If you are absent due to sickness for any length of time (even if it is 1 shift), then a self cert or sick note must be provided to cover the absence. This also applies to Bank Staff.

For absence between 1 and 7 calendar days, a self certification form must be completed; these can be downloaded from the staff section of the CHoC website. For absence of more than 7 days, you must obtain a certificate from your Doctor, stating that you are not fir for work and the reason why. These must be submitted to your line manager as soon as possible. If you absence continues, further medical certificates must be provided to cover the entire period of absence. If these are not received by payroll, then payment will be withheld until they are produced. Payroll will NOT chase these.
